Fill this out at each visit and leave with your customer before you leave. This form allows you to provide your clients with all you did on your visit.
It provides details about the chemical levels you tested and what you did to each level (ie. adjusted up, down, was okay and a place for comments).
It also provides information about what you cleaned, siphoned, etc.
It then provides an area for you to indicate any comments or suggestions in regards to the fish appearance, tank appearance, etc.
This form will be sent in Word and PDF format. It is completely editable in Microsoft Word and will come with instructions on how to open the file as well as how to edit it (including adding a logo).
