This form gives you permission to charge your client's credit card for all purchases you make up to a cap amount. If you are using your client's credit card, but do not have a photo ID to go along with it, you would show the merchant this form.
This form will be sent in Word and PDF format. It is completely editable in Microsoft Word and will come with instructions on how to open the file as well as how to edit it (including adding a logo).
