When you interview with a new client, this is the form you would use to document the services your client would like you to do for them as well as a description of the services. You also place the rate you will be charging as well as how they will be paying. This form also asks for home information just in case you will be entering their home while they are not there.
This form will be sent in Word and PDF format. It is completely editable in Microsoft Word and will come with instructions on how to open the file as well as how to edit it (including adding a logo).
